Did Your Tax Preparer Sign?

By law, paid tax preparers must sign both federal and state tax returns and include their IRS Preparer Tax Identification Number (PTIN) on all federal returns prepared for a fee. If your preparer did not sign your tax return by name (either typed or handwritten), marked it as “self-prepared,” or used only a business label instead of their name, please complete the complaint form.

Helpful information to include in your complaint:

  • Date you received service
  • Copy of the tax preparer’s business card (if available)
  • Amount you paid for the service
  • List of tax forms the preparer filed for you
  • Copy of your filed tax return(s)

Please provide as much detail as possible about the tax preparer you are reporting. If additional information is needed, a representative may contact you. Incomplete complaints or those without adequate contact information may result in your case being closed.

All information you provide, including your name and contact details, is kept confidential in accordance with privacy laws. Reports are sent directly to the California Franchise Tax Board Tax Preparer Enforcement Team.
